You need to have your home PC open to the internet if you are going to host your site yourself. Unless you are very careful with security doing this can be dangerous.
To enable your domain to be used on your home PC you will need to use a DNS service. This will resolve your domain name to your IP address. Once you have set up the DNS to point to your computer you need to edit the nameservers that are set up for your domain at your domain registrar.
Two free DNS services that are worth looking at are:http://www.everydns.net/ and http://www.opendns.com
